This seemed to happen shortly after the weekly reset – previously I had a weekly event badge equipped in slot #3 (so it was obviously removed with weekly reset) and had taken a moment to re-equip Slot 3 with a Medal of Orpheus.
However, upon swapping to a different Medal loadout (one of which is a triple Medal of Seasons, and the other is left empty) and then back again, all three Medals of Orpheus now render the same as each other.
