How often does this happen? When did it begin happening?
Always? Just saw it a few minutes ago while playing a battle in the faction assault event.
EDIT: Recalled that I was using Titan class with Chaos Storm talent. So it appears to be one of the randomly selected storms.
Steps to make it happen again
Unknown.
Usually the storm help number refers to the gem that has an increased drop rate, and 38 is the Blue Dragon gem. Maybe you got a blue dragon storm?

Some talents/weapons cause random storms. This is supposed to be limited to existing storms, in reality just about any storm still under development will slip in, even if it creates stone blocks. It would probably be helpful to see which team you were using, and which team you were fighting against.
