My belief is, one roll is determining all 3 chaos orbs instead of 3 separate rolls occuring.
3 different players collecting stage 9 and stage 11 rewards of ToD at once.
A dev will say it’s working as intended though

Correct it’s working as intended, it’s normal for you to get 3 of the same Orbs when using 3 Chaos Orbs, it’s also normal to not get the same 3.
It’s RNGesus at work.
