After the update, you need to finish an E12 run (it doesn’t count your previous E12 runs) to unlock E13 and so on and so forth.
Have you finished a level 12 run since the patch? You have to finish a run on one difficulty to unlock the next.
Previous level 12 runs apparently don’t count.

Edit: you are right. I have to actually finish an e12 run before e13 unlocks. I will change the title
Once you unlock a new difficulty level in 1 kingdom, it’s available in all kingdoms.
