What you were expecting to happen, and what actually happened:
In one of previous updates there was information that troop formations now saved between weeks for PvP. And for some time I believed it’s true. I saw some teams disappearance, but wrote them out as “not enough saved teams”.
Now I visited GoW after three weeks, and found all my teams missing. Including this pesky “Mech” team which is in one and only Winter’s Reach from time to time, and each time I decide to not save it in my personal formations, as usually I don’t need it, and, well, it’s saved, right?
So now I believe that “saving” works just as it was before this mentioned patch: i.e. the old team exists just before some other team will be selected instead. Obviously, in three weeks everything is erased this way. And it’s utterly wrong.
How often does this happen? When did it begin happening?
Always, since PvP started, I believe. Only I didn’t found it until now.
Steps to make it happen again

Put any team in PvP (I set them all. Many times.)

Repeat the next week when it will be erased inevitably.

Repeat for several weeks, until the first team appear again, and you need to set it again nevertheless.
