What you were expecting to happen, and what actually happened:
The Defender trophy appears to be bugged. I only do PVP battles in Central Spire and I have several defend victories but the trophy has not unlocked. It seems highly unlikely that none of those were in Central Spire.

How often does this happen? When did it begin happening?

Note - forgot to add the stats show that I have 136 defend wins in that region. So its definitely bugged.
