What you were expecting to happen, and what actually happened:
I expected to be told when a pet rescue starts, and for killing another pet gnome to have no effect. Instead, someone else in my guild triggered a pet rescue and afterwards I started a match and killed a pet gnome, which triggered the pet rescue pop-up, and I selected the pet I wanted, but it did nothing, because a pet rescue was already in progress, that my game knew nothing about.
Since one of the recent patches, we no longer get pop-ups telling us that there is a pet rescue happening, unless we exit back to the map screen, and now it’s clear that the game doesn’t trigger them until you reach that point, because it still lets you start new pet rescues and select your pet to rescue, even though it shouldn’t be able to do that.
How often does this happen? When did it begin happening?

Steps to make it happen again
Play PvP. Wait for a guildmate to trigger a rescue, then fight a pet gnome without exiting the battle selection screen, and beat the pet gnome. Select your pet and then exit to the map screen to find an existing pet rescue in progress.
